We can’t always prepare ourselves for the situations we are faced with in life, however when it comes to patients who have planned to have dental treatment with us here at the Dentaprime Clinic, there are two ways in which you can prepare.

Firstly would be to research the procedure and understand the process which you will go through as a patient. Secondly would be to ensure that any necessary pre-treatment is performed in advance by an experienced practitioner, riot to you travelling to Bulgaria. Here at the dentaprime clinic we always ensure that if a patient is due to have oral surgery or implant surgery, they understand exactly what treatment will be carried out, and what the necessary steps of the procedure are.

The other type of preparation isn’t necessarily down to the patient as an individual, but ensuring that any pre treatment that is required is undertaken by an experienced dental professional. For example, the prerequisite for successful implantation and a successful bone graft operation,  would be to ensure that the jaw where the operation is to take place, is completely free of inflammation or infection. It is essential to ensure that there is no infection within the jaw before the operation takes place. This is because the risk of peri-implantitis or an overall inflammation on the open bone is too large and why we here at Dentaprime will only operate when we know the situation and that the patient has had all necessary pre treatment.

In the majority of cases, any unwanted, broken or damaged teeth would have been extracted and the jaw left to heal for a period of 6-8 weeks, this is to make sure the area where the implant will be placed has healed. It is only in very rare cases, extracted teeth and implants are used simultaneously.

Before the dentist can make any decisions in regards to the cosmetic dentistry required, they need to discuss the medical history of the patient carefully, in order to rule out health problems.
Medical conditions can affect circumstances relating to the patients dental health and must therefore be considered; along with direct disease of the teeth and gums. The best example of a medical condition which can effect the dental health is diabetes.

Diabetes generally ensures that although certain interventions can be carried out, they may be limited. If such an illness exists it can increase the risk of periodontal disease to the patient . Acute periodontitis in turn prevents a possible implantation due to the increased risk of implant loss.

Risk Factors Prior to Implantation

The placement of dental implants can not be done unconditionally in every patient. Each individual case has to be assessed prior to treatment and both the dental and general medical conditions must be examined carefully ruled out, especially following impairments.

These include:
Bruxism
Disease and inflammation of the gums
Insufficient jaw
Poor oral hygiene
Pregnancy
Metabolic diseases
Alcohol, drugs, nicotine or drug abuse

” Before any procedures are undertaken, the health factors of the patients are always checked,” says Dr. Regina Schindjalova, head of the Dentaprime dental clinic.

“Only then we can give ourselves and the patients the assurance that the implantation will be completed to full satisfaction. If, during this testing process, we do discover a risk factor, we
remove it before we undertaken the implantation process. The artificial tooth root will eventually last a lifetime. ”

Bruxism & Gum Disease

Prior to implantation, a number of risk factors have to be excluded, so the implantation can take place safely and successfully. Two of these risk factors are bruxism and diseases of the gums:

BRUXISM:
Bruxism is defined as the unconscious gnashing of teeth. The most common trigger is stress.

“Bruxism is problematic, because of the strong pressure of the jaw it creates abrasion of the teeth and leads to loss of tooth enamel. It can cause small cracks that promote tooth decay and
other diseases.” says Dr. Regina Schindjalova, head of the Dentaprime dental clinic.

Initially this grinding of the teeth has to be resolved otherwise an implant cannot be recommended.

“The forces that are generated during the grinding is tremendous. Loads of several hundred pounds can affect the teeth and gums, “says Dr. Schindjalova.
“Especially in the first days and weeks after placing implants. The implants cannot tolerated such a big burden, because they only need to grow.”

The likelihood that an initial implant, then re-implantation is very likely in cases where the bruxism has not been resolved first and we always advise patients to have the necessary pre treatment before implantation. The ideal treatment for bruxism is a splint. This can reduce the effects of the grinding and can be worn over implants.

DISEASES AND INFLAMMATION OF THE GUMS:
Disease and inflammation around the mouth are a particular risk factor for any impending implantation.

“Inflammation increases the likelihood of implant loss,” says Dr. Schindjalova.
“It can always spread to the area where the implant was placed. As the implant
just sits fresh in the bone, it can settle the inflammation in this area, attack
the surrounding bone and lead to implant failure.”

Therefore, the jaw must be completely anti-inflammatory and disease-free, in order to guarantee a safe and successful surgery, prior to implantation. This is particularly true for patients suffering from periodontitis.

Could These Simple Problems Effect Your implants?

Before any implantation operation is performed, the situation in the mouth of a patient must be examined closely. There are several factors which may put the implantation operation at risk; these include:

Insufficient Space for the implant, if the jaw is not high or wide enough.
Poor Oral Hygiene

NOT SUFFICIENT SPACE:

The requirements for placing an implant within the jaw are very specific. A certain height and width of the jaw is essential to achieve primary stability. If the bone is not high enough, the implant would protrude from the jaw, and would neither grow nor be very useful. If the bone is not wide enough, the implant would not be able to hold onto the sides and therefore would not be stable. In the worst case scenario a bone augmentation and alternations to the bone are needed before the implants can be placed.

“By using the modern technology that we have, we can estimate exactly how high and how wide the jaw bone is.” says Dr. Schindjalova. “Where there is no substance, we perform a bone augmentation
before implantation.”

For this bone formation procedure, bone substitute materials can be used. In some circumstances the body’s own tissues can be used to prepare the jaw for the necessary implant.

POOR ORAL HYGIENE :

The importance of good oral hygiene is essential as poor oral hygiene can be an equal double jeopardy.

“The likelihood is great that the oral hygiene of the patients will not improve immediately. Even if the necessary measures will be undertaken by a prophylaxis assistant, “says Dr. Schindjalova.
“It is important that the patient undertake excellent oral hygiene methods for themselves and that this procedure is maintained, only then can an implant be used successfully. ”

Once a patient has learned to take the proper care of their teeth, they should also be educated about the consequences of poor oral hygiene and introduced to the techniques of tooth cleaning. Only after regular, positive performance reviews can implantation be undertaken. Once a patient has a good oral hygiene programme nothing stands in your way for receiving top class treatment.

INADEQUATE SUPPLY:
When considering implantation it is also important to treat the remaining teeth . For example, if there are insufficient crowns or fillings defective in the jaw, these should be treated prior to the implant so that they are in immaculate condition. Therefore when the implants have been placed and all the remaining teeth treated, patients will leave the clinic with a wonderful, bright new smile.

A Motorcycle Accident leads to a Beautiful Smile.

Melanie V. was  24 years old when, during a motorcycle accident, five of her upper teeth were knocked out. Her dentist advised her to have implants at this time, however, this option was too expensive for the young woman. Instead, she opted for a cheaper alternative: a partial denture that is secured with clamps onto the remaining healthy teeth.

 

Initially, Melanie was satisfied with the denture as she wanted to save on the price of implants, however the young woman was also clear that she did not want to spend the rest of her life with a removable prosthesis.

After about a year Melanie V. noticed that one of the healthy teeth on which the clip was attached had started to wobble.

       “I could not imagine that the problems would  be due to the denture. Until that time, it had  always sat well and never caused me any problems.” she recalls.

When she visited her dentist, his diagnosis is made quickly: the tooth had actually become loose due to the loading of the denture. “The prosthesis was indeed a good alternative solution, but not ideal to solve the problems with the existing gaps in the teeth.” said Dr. Regina Schindjalova, Head of Dentaprime clinic in Bulgaria.

In some cases dentures can cause damage to the remaining healthy teeth. When wearing the denture, it may well occur that teeth become loose and eventually have to be removed, due to the minimal friction layers of the tooth enamel.

Melanie V. also received information regarding the possibility of bone loss. When the jaw no longer has a tooth, the surrounding bone can recede causing further long term damage.

     “My dentist informed me that the body breaks down tissues that it does not need and       that’s exactly what happened with my jaw bone. The prosthesis had not burdened the       jaw and thus the bone itself had receded. “

At Melanies’ request her dentist calculated a cost estimate for bone augmentation and implant placement. The cost estimate that she received turned out to be far higher than the young woman could afford, even taking into consideration her savings. At this she was upset and decided to look for alternative treatment.
